Total cost for each line item will compute automatically.)
- Provide your e-mail address
so the detailed list of these items will be e-mailed to you
and your contact name and phone so we can reach you if we have
questions about your order.
- Use the button to add the
total cost of the items you listed on this form to our website
shopping cart. ( This
will create a single item "CATALOG ITEMS" added to
your shopping cart with a cost equal to the total. NOTE:
Separate detailed information for each of the items will not be
displayed once you leave this page).
- TO
COMPLETE YOUR ORDER you must click
"CHECK
OUT" before you leave our website.
BUT, AFTER YOU FINISH ORDERING ITEMS WITH THIS FORM you
